1. Participant and signing authority
The signer confirms that the participant information is accurate. An adult participant signs for themself. If the participant is a minor or is not legally able to consent, the signer confirms that they are the participant’s parent or legal guardian and have authority to give participation and medical permissions and to make the signer’s own agreements below.
2. Fitness, rules, and disclosure
The participant will follow HAC, coach, facility, equipment, and safety instructions; use required protective equipment; act responsibly; and stop and report an injury or unsafe condition. The signer will disclose relevant restrictions or accommodation needs to HAC before participation and will not allow participation when the participant is unfit, injured, ill, or impaired. HAC may refuse or end participation where reasonably necessary for safety or conduct.
3. Inherent and other risks
Hockey and athletic training involve foreseeable and unforeseeable risks, including high-speed movement; falls; collisions with people, boards, nets, pucks, sticks, skates, equipment, or surfaces; cuts; bruises; sprains; fractures; dental or eye injury; concussion or other head injury; cardiac events; communicable illness; equipment failure; facility conditions; travel; acts or omissions of participants or third parties; serious disability; and death. Risks may arise even when rules, supervision, and reasonable care are used.
4. Voluntary participation and assumption of risk
The signer understands the nature of the Activities and voluntarily chooses participation. The adult participant knowingly accepts and assumes the inherent and other risks described above. A parent or guardian acknowledges those risks and voluntarily permits the minor participant to take part.
5. Emergency care
If the signer cannot be reached, the signer authorizes HAC to seek reasonable first aid, emergency transport, and medical care for the participant. The signer remains responsible for resulting costs not covered by insurance. This permission does not require HAC to provide medical services or guarantee any particular response.
6. Release by an adult participant
To the fullest extent permitted by Ontario law, an adult participant releases and agrees not to sue HAC, its directors, officers, staff, coaches, volunteers, contractors, host facilities, event partners, sponsors, and their respective representatives (the “Released Parties”) for claims arising from participation in the Activities, including claims connected with ordinary negligence. This release does not exclude liability that cannot lawfully be excluded, including where applicable gross negligence, reckless or intentional misconduct.
7. Parent or guardian terms
A parent or guardian gives informed permission for the minor participant to participate and, to the fullest extent permitted by law, releases the signer’s own claims against the Released Parties arising from that participation. Nothing in this agreement is intended to waive a minor’s rights where Ontario law does not permit a parent or guardian to do so. The parent or guardian agrees to reimburse reasonable losses caused by materially false information or the participant’s intentional violation of safety rules, to the extent permitted by law.
8. Personal property and insurance
HAC is not responsible for lost, stolen, or damaged personal property except where liability cannot lawfully be excluded. The signer is responsible for appropriate health, travel, accident, and equipment insurance and understands that HAC does not provide personal insurance unless expressly stated in writing.
9. Governing law and severability
This agreement is governed by Ontario law and the applicable laws of Canada. If a provision is found unenforceable, it will be limited or removed only to the extent necessary and the remaining provisions will continue. The electronic acknowledgement associated with a purchase or booking is intended to be the signer’s legal signature and a record of acceptance.
